      The debt in question was suppose to be paid by my insurance company for a hospital bill, however the doctor did not file the paperwork in time to my insurance company to pay the debt. He sold the debt, and now two debt collection agencies are in question regarding the debt. They both refuse to honor my request to remove the debt from my credit profile, after sending them both debt validations to prove the original contract was with them. The original date of the debt stems from the Sept 2017, in the amount of $1,401.00. This debt is impacting my credit score at no fault of my own.

      Cascade Receivables Management, LLC (CRM) has received notice of the complaint filed by this consumer and CRM takes all consumer complaints seriously and strives to resolve all complaints in a quick and efficient manner. Cascade Receivables Management, LLC is not a data furnisher as defined by the Fair Credit Reporting Act and does not report any debts to Credit Reporting Agencies.
      CRM has investigated the consumers concerns set forth in the complaint.
      On or around April 3, 2020, Cascade Capital Funding,LLC purchased an account listing ******************* as the guarantor and patient with a last known address of ************************************************.
      The original creditor is Belle ****** *************** LLC. This is the name of the billing company for the ************************************* Emergency Room where ************ received services on September 30, 2017.
      The insurance listed on this account is Medcost, Inc.policy number ending in *****. After insurance was applied the remaining balance of $1,401.00 is listed as patient responsibility.         
      On May 9, 2024, CRM mailed the validation of debt to ************ at the address listed in her complaint.

      I went to an emergency clinic in 2018 and insured prior to arrival and upon arrival that everything being done was covered thru my ins carrier. The only medical treatment I received was an X-ray. They informed me nothing they could do for bruised ribs. I received a bill for some 10k dollars and was told the then Dr. ** duty was not in my network and was billing separately.I took all my information to my HR **** who resolved the matter. I got a call 10 mins ago from Cascades Rcvbls trying to collet the debt. I informed them my employers HR department resolved the matter and for them to contact that employer about any questions.I feel this is a harassment tactic to try and squeeze money out of me on a debt that has already been resolved and is very disturbing.

      I am rejecting this response because:

      They are suggesting I did not give a reason for them to Contact my ex employer when I directed them specifically to contact ************************ HR department.

      ***** is the contact person. Their phone number is ************.

      Prior to going to their clinic and upon arrival I specifically asked was everything covered under my insurance and was told yes. After leaving with no notice of money owed I received a letter stating I owed money. I went to the clinic and asked why and was told oh well the doctor that saw you wasnt in network.

      I advised my employer of the circumstances via HR department and they said they would handle it. Thereafter I was informed by HR 
      It was resolved and did not need to worry about any charges.

      This company needs to contact my old employer for records regarding this claim as it was handled through them and should still be on file.

      Cascade Capital Funding first contacted me March 14, 2022 informing me that I owed a balance of $1,097.20 to original creditor Code 3 Emergency Physicians. I explained to the representative (********) that in October 2019, I had set up a monthly automatic payment plan with Code 3 that authorized them to charge $100 to my **** card until the amount was paid. Cascade Funding called again in May, I spoke with a different representative (*****?). We had the same conversation that I had had with the representative in March. I told her the bill had already been paid and that I did not own Code 3 Physicians any amount.The second time Cascade Capital Funding called I reached out to Chase ****, April 2022, requesting that they send all monthly statements dating back from October 2019. About two weeks later I received all statements from October 2019 until November 2021. What I discovered, upon inspection of the statements, not only did Code 3 Emergency Physicians charge $1,097.20, they actually continued to charge $100 monthly for a total amount of $2,000. By this time, they had gone out of business and we (*****) was unable to review and refund my account because of the timeline.Cascade Capital Funding called again, June 2022 I was directed to the manager, whom I believe went by the name of ***, who clearly stated You do owe Code 3, we are here to collect the amount owed, $1,097.20 and we are trying to make it easy on you so how did you want to pay? I believe I asked him whether he heard what I was saying, which was, the bill had already been paid. I also told him to give me about a day and I would send the verification proving that this bill had been paid in full.To resolve this matter, June 9, 2022, I sorted through the **** statements and charges and sent Cascade Capital twelve **** statements that clearly showed: SHA*Code 3ER $100. I followed up with Cascade since they did not follow up with me. I inquired whether they had verified the information. I was told more than once that they were still investigating. I did not hear anything for a while. Their next call came, August 2022. I spoke with the manager again and reiterated that the total amount was finalized, last charge, May 2021. He then agreed with me, Every word you are saying is true, but this is a different bill. There is no other bill!To this end, Cascade Capital Funding refused to accept the verification that the amount had been paid in full a year prior to when they first contacted me. Instead Cascade chose to try and intimidate in an effort to collect on an account that already been paid. they would not acknowledge the error on the part of the company (over charge) Code 3 that was no longer in business.I received a letter Wednesday, February 22, 2023 from: *************************: Account Summary Current Creditor: Cascade Receivables Management Reference # ********** Original Creditor: Code 3 Emergency Physicians Balance Due: $1,097.20

      As indicated in the initial correspondence, the amount owed had already been collected and finalized a year (2021) prior to Cascade contacting me. Maybe incomplete documentation is what's left when some companies go out of business. Nonetheless, this auto payment of $100 between Code 3 Physicians and ********** was set ** in October 2019 and the last payment charged to my account was May 2021. Should you need documentation of payment, do not hesitate to contact me.

      In response to Cascade Capital, several details are questionable. The paragraph that begins, "The original creditor was contacted..." How is this possible when they have been out of business at least since October 2021? Also, the paragraph that begins, "On 8/30/2022...balance still owed." Last item, that begins, "A review of all available...is still unpaid..." My question is this: What is the account number assigned to the patient or associated with this charge Cascade still claims I owe? 

      In response to the last two items above: Prior to Code 3 Emergency closing their doors, I received two bills: doctor and facility. The insurance paid a portion to the doctor and I paid the remaining balance, October 26, 2021. Yes, I have the receipt and possibly a screenshot that proves the doctor's portion was paid. 

      Concerning the facility, my portion was $1097.20. Code 3 not only charged this amount, they changed a total of $2,000 before I realized a monthly charge of $100 was still being charged to my **** account! The proof is on my Chase statements beginning October 2019, when I initiated automatic monthly payments to Code 3. You must remember I did forward the **** statements verifying these charges. To this end, Cascade still has the audacity to make the statement that the disputed amount has not been paid!

      I need for Cascade to understand two truths: both bills were paid prior to March 2021, the data proves it. I also need for you to understand I will not send any amount to you, a third party collector or any other fraudulent company. Finally, whether Cascade or an affiliate, *************************, please do not contact me again.

      11/28/22, I was contacted by Cascade Capital Funding LLC, for an outstanding bill from ********************. This bill was for ******, on a total charge of *******. After contacting them I spoke to a ***************, General Manager. He tried to say this was a valid debt, however I had **************** at this time. This bill is from a ******************** with an account number of *************. The hospital is not in business anymore. I wrote a certified letter for information, in the letter I included the EOBs for the visit in which they were referring too. I also asked if he was aware of the ***** Fair medical Billing law, in which he stated he was. I explain that this bill is over 2 years old and I want proof that I was actually billed for this. Which he could not produce. 12/28/22 I got a letter in the mail that was a bill for ******. I called them again only to be told the case was closed out. I asked for a letter stated this claim. 2/6/23 I still had not received a letter. I called and was put with ****** to be told my case was not closed and still had a debt of ******. I explained that that is not what I was told and she even confirmed it was in the notes on the account. I called back again 2/7/23 and tried to reach ****** again, only for her to start the phone call then abruptly cut me off an pass the call to ******. ****** he said I have a balance of 125 dollars. He told me he verified the information with ***** *************** I ask how he did that since the hospital is closed. He told me to figure it out. I told him that was unacceptable for a company to try and collect an alleged debt to figure it out. So I asked for the complain department along with his supervisor. *** who I spoke to the first time was the supervisor. ****** would not get complaint department. After speaking to Ray it was the same thing, they are unwilling to provide any information that I was billed correctly or provide information of who the bill was actually verified with.

      This collection agency is using debt collection companies like Elevate Recoveries, LLC and ********** to do a shell game to pass back and forth bad medical debt.In November 2021 I received a letter from Elevate Recoveries attempting to collect on alleged medical debt from a bankrupt ER business. I promptly sent a letter via certified mail asking for proof of medical debt.(1) I had no communication from this company for months and then was contacted by a debt collector on 6/14/2022 acting on behalf of Cascade Capital Funding trying to collect on the medical debt. I requested proof of debt again, and filled the complaint below (1) with BBB. Elevate Recoveries wrote back on BBB that they had requested the account be closed on 5/24/22 and a request was sent to the credit bureau to remove this account. I was then contacted by ********** on 7/12/2022 to collect on said alleged medical debt on behalf of their client, Cascade Capital Funding. I requested proof of debt again.       CCF has done an investigation as to the consumers concerns set forth in the complaint. Our investigation as produced 
      the following: 
      On 10/29/2021 CCF purchased a portfolio of discharged consumer accounts.
      Within this portfolio is an account that lists the obligor as *************************** with a last known address of 3230 
      *****************. ********, ** 77584
      The original creditor listed on both accounts is First ***** Hospital (FTH). This is the billing group for the 
      physicians at *************************** (FCEM)-********, Silverlake.
      FTH and FCEM are separate entities, and each maintains their own billing records.
      The Date of Service listed on the account is 12/4/2016
      ************* provided at the time of services is listed as AETNA
      The Incident amount listed on the account is $843.88
      The current outstanding balance on the account is $843.88
      On 11/16/2021 this account was assigned to Elevate Recoveries, LLC (ELE) for collection.
      ELE records show that there was no direct communication with this consumer. 
      ELE records show that on 12/22/2022 they received a request for an Itemized bill for this account.
      ELE placed the account in DISPUTE status and submitted a request to TransUnion (a credit reporting agency) to 
      list the account as DISPUTED. 
      ELE contacted CCF to request a copy of the Verification of Debt (***). 
      CCF contacted the original creditor and requested a copy of the ***
      CCF records show that the *** was received on 3/24/2022 and forwarded to ELE to send to the consumer. 
      On 5/20/2022 CCF recalled the account from ELE for non-payment.
      On 6/6/2022 this account was assigned to ********* (AVE) for collection. 
      AVE notes show that the consumer contacted them on 7/12/2022 and informed them that she was not aware of 
      this bill and had not received anything regarding it. The AVE agent informed the consumer that they would mail 
      information to the consumer at the address on file. 
      AVE placed the account in DISPUTE status and ceased all collection activity on the account until the *** could 
      be provided to the consumer.
      CCF notes show that on 8/8/2022 the *** was forwarded to AVE to mail to the consumer. 
      CCF received confirmation from AVE that they received the *** and mailed it to the consumer on 8/8/2022.
      Despite the availability of the ***, CCF has opted to recall this account and close it as a REFUSAL TO PAY.

      I called United Healthcare & spoke to a rep on 11/16/2021 after receiving a **** from over 4 years ago, for emergency services, which I gave the hospital at the time of service my Tricare Information, because Im retired military, & worked as Contractor, & gave ER 1st Choice my insurance for UHC for the date of service and Tricare. After 4 years, I get a **** from a collections company for a facility that no longer exist telling me that balance $1628. payable to First Texas Health. ACCT# THH **********. The creditor now that's trying to collect on this **** is a collections company called Cascade Capital Funding, LLC. Acct #**********. As stated, I was bleeding out and I was told by the VA and UHC that this facility took both insurances, Therefore; I would of had no payment due. I was hemorrhaging and lost a lot of blood, and was told that I was miscarrying, when I did not even know I was pregnant. Again, I gave both insurances, in order for the clinic to see me, and now I am being told by rep ******* that UHC was submitted at the time as I stated, but she does not know why the **** was not processed correctly leaving me outstanding $1628. Told Tricare was billed as primary and she shows in the system where the claim was trying to go through but she believes the way ER 1st Choice put the **** in w/Tricare as primary, may have led to the problem. On 11/16/2021, I called UHC and Tricare paid out, & UHC told me I had 2 yrs to dispute matter or I'd be liable for a **** I did not know exsisted & even though I just got ****. I did my part, providing clinic all insurance needed for my dual insurance. I was told by the Cascade collection DPT that the facility has went out of business and they are now collection of their old bills. I asked for proof as to why **** submitted incorrectly and told its hospital thats out of business error, they are just there to collect. UHC ID ********* Group: ****** ***** *************************. Tricare paid ****. total **** $2765. I want UHC pay ****.
