I contacted ********* (case worker on 2 ***** at Doctor's Hospital of Augusta, **) in reference to getting a copy of VA Form 21-2680 (Examination for Household Status or Permanent Need for Regular Aid Attendance). During this conversation, ********* informed me that she was working alone this week due to the absence of ******* ( case worker). I was informed by ********* that she would email me the form shortly. I followed up with her on June 12 and left a voicemail on ************, she did not return the call. I followed up again on June 14 and left a voicemail with no return call. I contated ********* again on Monday, June 16, she stated that she would "look for the form and get it to me". I did not receive the form. I sent ********* a text message on June 20 again following up and requesting a copy of the form. She has yet to respond. I left a message with the outpatient coordinator ************************* on June 20 and I have not received a return call. I also left a message for ******* (Director of Case Managers) on June 20 and I have not received a return call. Additionally, my father, who I am trying to assist contacted ********* several days after my mother was discharged from Doctor's ********************** in April. He was also requesting a copy of the form. ********* informed my dad that she would forward him a copy of the form. My dad has not received the form. We have expressed the importance of receiving a copy of this form to get assistance from the VA as it relates to needing assistance for my mother. My father and I have tried endlessly to get a copy of this form from *********, however, she is not responding to any of the requests.Initial Complaint

I went to doctors hospital ER with a dislocated knee in extreme pain and was treated like trash received 1 pain pill hours after being left in a back room on a hospital exam bed incorrectly where the feet is supposed to be in the bed my head was there ....I cryed to get them to turn me the correct was they did 9 hours after X-ray they claimed the dislocated knee went back in place an they were sending me home not meds just to call my knee dr for a follow up I should have been admitted a knee taken care of because yo this day i cannot stand on my knee because it's still dislocated rude nasty Emergency room admission black lady and Emergency room doctor who told us his name was **** which I knew wasn't true he left no notes in my patient portal online so I couldn't get his real name I was treated like trash and I would like an apology an refunded for this day and previous visit handling my dislocated knee.Initial Complaint

I went to the ** June 2021, however after waiting over 3 1/2 hours in excruciating pain and not being seen I went to the front counter and advised the lady I was leaving to try another hospital being I had not been able to make a bowel movement in over a month at that time. The lady stated she would let the appropriate person know I was leaving. The only thing I received was a wrist band and received a **** for $392. I advised the billing department when I received a **** and was advised to disregard the ****. I go to the mailbox on Monday and I received a **** from an collection agency for the same **** I did not receive service for. I am writing in hopes of assistance being I was never seen except by the front to put a wrist band on and told me to wait. I finally left so I could find another provider that would see me faster being the pain I was in was intolerable. The **** has my old married name of *************************** *****. I am no longer a ***** for 13 years now.
