Hello, Edward Health, now called Endeavor Health, has just now billed me in March of 2024 for services rendered on 10/28/22. I have contacted BCBS (I have attached their statement), as well as the statement from Edward/endeavor health. I have emailed ******************************************** with no response back and have waited on the phone over 322 minutes to 'talk with someone', with someone one time answering the phone and then hanging up on me after giving them my guarantor number. *************). The line items match up to $16,692.00. I have paid now $952.22 of the original $752.22 that was due. It looks like Edward/Endeavor Health has slipped in another charge not included in any of these of 'Other imaging services of $2188.00? and never submitted to the insurance.the attached BCBS clearly states amount billed $16,692.00 other carrier payment $0 discounts and reductions -$12,383.75 health plan responsibility - $3,556.03 you may owe provider $752.22 I am now getting a bill in 2024 for $2225.66 as this does not line up with any documentation we have especially from BCBS who should have been billed? bcbs claim number 02022304606Y7560H-01 Service date 10/28/2022 BCBS of Texas advocate customer number ************Initial Complaint

I visited Edward Health Ventures ****************************************** on September 20th 2021 for an annual physical. I did so in order to schedule my annual blood work and renew my medications which require an in-person visit. I told ************************************************ this when she entered the room after exchanging pleasantries. She asked me if I was having any problems and I responded no, that everything was good. She then started reviewing my chart and asking questions about my current diagnoses; I assumed so that she could authorize continuing my current medications. She then performed a brief physical exam, set up renewal of my prescriptions and authorized my scheduling annual bloodwork. I then left the examining room.I then received a **** for an office visit for $159.47 instead of the expected no charge annual physical which had been the results every year at the same practice for over 10 years. I am disputing this charge. Complete problem description in attachment
