This is for the Lake County Physical Therapy LLC ***************, **, Moon Lake ******************************************************* location which I visited for 19 sessions from August 16, 2022 to November 01, 2022 for physical therapy. On August 15, 2022 I texted with a Billing representative by the name of ***** and provided him my insurance information. He checked it and he replied that I will need to pay $35 per visit (screenshot attached). On October 10, 2022 I realized that there is a billing error I saw on my insurance portal that they put charge of $360 per visit and that I need to pay $139.63 of that after insurance. I contacted ***** about it and he told me again $35. He said that he will check again and contacted me few days after and said it's an error, that they billed the wrong amount to my insurance and that they will change it to $35 as agreed. He told me that my balance at that time was $245. I didn't check with them anymore because I never received any statements directly from them. If I did, I could know that they lied about how much I owed them. The first statement from them was on February 9, 2023, when they had closed my account. I received a bill for $2,882.47. ***** didn't want to pick up any phone calls from me. I reached out to ***** and after a few weeks he said that ***** made an error when he told me $35 per each visit but that I still need to pay $2882,47 and not $665 as agreed. When I finally was able to talk to *****, he verbally attacked me and said that even though they made the error, I still need to pay the full amount. I asked to talk with a manager or their boss, but they declined. They said the boss doesn't want to talk with me. They wanted to give me discount of 20% that is not even nearly fair. I did offer them to pay 50% to meet them halfway, but they refused and they said they will put me on collection for the full amount on 90 days from billing date. From my view they done fraud to over charging me and over charging insurance.
