Called and spoke to representative on Feb, 19 2024 to make payment of ********** received for services provided. I gave them my FSA Health Account debit card number to pay bill. I asked for them to send me a copy of receipt for billing purposes. I have spent approximately two weeks & numerous phone calls to no avail. They acknowledge that the invoice is now paid.....but can't seem to send me a receipt showing that I paid this bill. My FSA account is asking for proof of payment...and I can't provide at this time. ACCU keeps telling me that they sent email, but it isn't true. They claim that an associate in billing will send me a copy of receipt in the mail. Accu Reference, a company who provided us with an in office phlebotomist, and with whom we had a professional relationship for 5 years, recently sent us a collection bill for COVID tests that were never performed. We believe they are doing this to retaliate that we chose to work with another lab, Quest, who is their direct competitor. They also accused us of cashing a check from our insurance which we NEVER did.They are threatening us daily with court action and credit reporting, which is unfair and unwarranted.Initial Complaint

My wife went to Dr ******* ******** D***** on 12/08/22 in ********** ** for follow up visit. I have to use ********** for her insurance. I received a bill for 4/1023 for amount $1,056.43 of lab works. They claim total amount $2,210.66 to my insurance, insurance ** only paid $214.52. How much does urine culture **st. Owed unpaid travel expenses for distance traveled using own personal vehicle. Was given false information regarding submittal of travel form by appointed employee.
