      May 23, 2022.- Cat brought in for euthanasia, was removed from waiting room at ****am. No further contact until 11:00am when I was taken to room where another cat was euthanized.I paid $419.15 for euthanasia plus surgery (NOT DISCUSSED) for $812.00. The credit card used for the surgery was one I had used there 2 years ago, not the one I used that day for the euthanasia.

      The client presented her pet to the hospital on September 24, 2022. The inital invoice of $812.00 was for services that were approved and paid for by the client. The client was going to transfer her pet to a 24 hour facility, but elected euthansia and aftercare at Taconic Veterinary Center instead. That invoice totaled $419.15, for which she paid.  Tell us why here...

       I never approved any medical treatment for my dying 17 year old cat.  I had brought her in for euthanasia only.  Taconic

      Vet Center sent a young woman to pick up my cat and take her away from me without saying a word.  They apparently thought they had some other cat who needed skin treatment, not my cat!

      On September 24, 2022, *************************** had an appointment scheduled for her cat ****** to be euthanized but changed the appointment to an examination prior to arrival. ****************** arrived at 9am and after the examination, a treatment plan which included the exam, recommended blood work,and a blood pressure with the cost was created, approved, and signed by *****************, (refer to estimate number *****).  The bloodwork findings were discussed with ****************** and the owner elected to go to a 24-hour veterinary hospital for treatment. The owner paid for the exam, laboratory work and blood pressure, with credit card ending in ****, (please refer to the copies of the itemized invoice and signed credit card slip). After the owner was informed of the cost at the 24-hour veterinary hospital, she elected humane euthanasia and communal cremation at Taconic Veterinary Center.  These services were itemized on the second invoice, resulting in a second credit card transaction, for which ****************** paid with credit card ending in ****,(please refer to copies of (itemized invoice ****** and signed credit card receipts). Taconic does not keep any credit cards on file for security purposes on the clients behalf, only the last 4 digits of the card, supplied by the card holder, is recorded in our payment module. Please refer to the screen shot that furnishes these numbers. ****************** did call looking for the carrier her pet arrived in, and it was explained that Taconic returned the carrier to ***************** however, the staff members did check to make sure it was not left behind. The medical records and laboratory results have been furnished for reference of medical services provided. Please let Taconic know if there are other concerns that we can clarify.
