      After seeking ADHD coaching online, I found the Affinity Center’s website, which lists “Coaching” as a service offered for adults (please see attached screenshot). I first spoke with office staff on the phone on December 10, 2021. I told them that I was seeking ADHD coaching to help with executive functioning issues (planning, organization, time management, etc.). They said they could help me with this, and I scheduled an appointment, that had to be changed due to an issue with them switching systems. I was assigned to Robb M, LPC since I did not request a specific provider. My first appointment finally occurred over the phone with Mr. M on January 19, 2022, at 1:30 p.m. This was an intake appointment lasting over 1.5 hours. He conducted a review of my life & medical history regarding how ADHD has negatively affected me. I again clearly articulated that I was primarily seeking ADHD coaching to help with my executive functioning issues, and he said he could provide practical solutions. I was clear that I have done a significant amount of traditional therapy and was not seeking this. Throughout the course of 4 appointments, I was told repeatedly by **** and an Office Mgr. (Terry P) that I would receiving ADHD coaching. I never received any at all, even after I pushed the issue, which meant I struggled with daily functioning. (see attached screenshots of email with *****) This wasted around 4 months of my life during which I could have received the treatment I need. Additionally, I'm unemployed partially due to my disability and am currently living with family while I take an online course to enter a new career. My family had to pay out of pocket for me to NOT receive the promised services. I would like a full refund for what we've already paid ($275) and to not be charged the remainder (around $600+).       My daughter is covered by my medical insurance, and I pay her medical bills. Before I made an appointment for my daughter at Affinity Center, I was given a schedule of fees and told by the billing office that I had to pay up front. I was told that they do not file insurance, but they would give me all of the information that I needed to file on my own, including a diagnostic code. I paid $250 for a full, 1 hour session. When I requested the information needed to file the insurance claim, they would not provide me with the required diagnostic code. They said that 1 hour was not sufficient time to make a diagnosis. If I wanted a diagnostic code, then I would have to pay for additional testing and more visits. The 1 hour session was very disturbing to my daughter, and she decided to switch to a doctor in a different practice. You can bet that the new doctor had a diagnostic code after a 50-minute session. It’s certainly possible for mental health professionals to provide a diagnostic code after 1 session for insurance purposes. Affinity Center’s assertion that they can’t provide a diagnostic code is not consistent with what the billing office told me prior to the first appointment. It’s clearly a scam to hook us into paying for additional services through them. I am seeking a diagnostic code for insurance purposes. My daughter has signed a release form, and the Center is legally permitted to speak with me about her case.
