My wife and I went through two rounds of IVF in 2021 with Ohio Reproductive Medicine. During the second one, we were charged for an optional ICSI procedure in the amount of $2,357. We were never told of the amount of this charge before it was performed and we did not authorize it. We have repeatedly asked Ohio Reproductive Medicine for documentation demonstrating otherwise, and they have not produced it. The deposit for the July IVF was taken over the phone on June 22nd for $5,572.29, consistent with the first round of IVF earlier in the year. The first time we saw the cost sheet including the ICSI was when ******** ***** from billing emailed it to us on November 16, 2021 (after she erroneously sent us confidential information from another patient on that same day). We don't see anything that indicates this was provided to us before then or that we consented to those charges. Further, ***** ***** from billing sent us an email on November 4, 2021 stating, "the billing department fully acknowledged that ICSI was not calculated into the deposit for the second cycle and was an oversight." Due to the large increase in cost for the ICSI (2,357/7,113.62 = over 33% of the cost of IVF), shouldn't this charge have been explained to us and our approval received before it was performed? We do not see any documentation indicating it was. We have reached out repeatedly for clarification and proof of authorization for this charge and have been ignored, bullied, and threatened as Ohio Reproductive reported our outstanding balance to a debt collector for referral to the attorney's office for garnishment of wages and a strike on our credit report. We are asking for a refund of the ICSI charge of $2,357 by Ohio Reproductive Medicine and a correction to our credit report. The doctor in charge is *** ***** ******* and the office manager is ******* ******.
