      I wish to complain about a referral fee charged by Dr. ***** of Primary vision and Blomberg eye center in Newark, Ohio.

      My regular eye doctor retired during the pandemic. I went to Dr. ***** of Primary Vision in Newark, Ohio. He said that I needed cataract surgery. I ask him for a recommendation that he supplied. He did not tell me I would be charged for the recommendation.

      When I went to the Bloomberg Center for the surgery, after a long wait, a person asked me to sign a number of forms she had on a clip board, She announced what each form was for. I failed to read the forms. One form was for consent to bill. She said there was a separate charge for Dr. *****. I asked if he would be there also? She said "No, it was a $200.00 referral fee." I was charged $200.00 again for the second eye. I payed a total of $400.00 for a single referral.

      I did sign the agreement, so I have paid.

      This is letter regarding case # ********, **** ******. This letter is to inform you that **** ****** was a Patient at Bloomberg Eye Center. On 8/22/2022 **** decided to do the Upgrade Lens, during a call with ****** to giver her Surgery date and arrival time and she also discusses what option of lens that the patient would like. **** Picked the Toric lens which the cost of this Lens is $1495.00. Since we Co-Manage with Dr. ***** and she made an upgrade The Toric Lens, Dr. ***** would get paid $200.00 of $1495.00. We have all Patients Make out checks to Co-Manager Doctors and we send the check to the doctor. On 8/29/2022 **** did continue to do The Toric Lens which paid $1,295.00 was made out to Bloomberg Eye Center and the $200.00 Co-Manager fee check was made out to Dr. *****. So she still paid the total of $1495.00, just in Separate Checks. Attached you will see a copy of the agreement that she signed which means she did agree with the Co-management with Dr. *****. It is not a referral fee whatsoever. My Billing Department did call her on 11/7/2022 Monday and tried to talk to Sara, She did not want to talk about the mater at hand and stated why are you even call me about this, this has to do with Dr. *****. My Billing manager tried talking with her but she was not compliant to listen.

      I did not write checks to Bloomberg. They were paid by credit card. When I asked if Dr. ***** would be present at my surgery, I was told " no this is his referral fee", I have not proof of this verbal exchange.

      When their billing department called me, I ask for a written statement of their position. They did not comply with this request. This is the first I became aware of the term co-management. I failed to read the forms I was given.

      I do not wish to pursue this matter further. I just wish that I had chosen to be a patient of Bloomberg Eye Center and Dr. *****.
