1) On September 14th at 3:00PM, I had a new patient and first office visit with Dr. ***** **** to review symptoms such as tightness/pain in the chest, shortness of breath, difficulty breathing, heart palpitations, and very high heart rate/abnormalities. Additional testing with scheduled for me which included, CT scan, blood work, Holter monitor, etc. Please see the document attached for further information. 2) On September 14th, I was given a Holter monitor and instructed to wear this for a week (September 21st) and send it back to the hospital for results to be reviewed. I sent this back on September 21st through FedEx as instructed. 3) I had blood work done at UH Ahuja Medical Center the week of October 24th, which was sent to Portage Medical Center and the results were given to me and stated that I had low potassium. 4) I had my CT scan in the Radiology Department at Portage Medical Center on October 21st at 1PM and was there for 5 hours. No summary was given to me afterward. 5) On October 26th, I had a follow-up appointment, and the receptionist was very rude to me when I showed up for my appointment a few minutes late due to working in Beachwood and getting caught in traffic on my way down to my 4PM appointment. She was very short and unprofessional to me and made me pay $45 for a follow-up appointment, even though this appointment was just for the results and there should not be a co-pay for a follow-up appointment. The doctor had zero results for me and asked if I had the testing he ordered done, I did. He didn’t understand why he didn’t have any of the test results back yet or anything to review with me. I had no results besides the blood work and still had to pay for this appointment. I have zero results besides the blood work and low potassium. This hospital and departments are negligent of my health concerns and have ignored my and my mother's phone calls/concerns. I 100% believe they are lying and lost my test results. I want this investigated
