            I am requesting a refund from Dr. Ben Alderdice Oral Surgeon. My Delta Dental Insurance Maximum approved fee for an extraction with DOS: 3/8/2023 was $291.00, allowed amount was $221.00, my plan paid $187.00 to Dr. Ben Alderdice. I paid $260.00 in cash. I have enclosed a copy of the EOB and a copy of my receipt. I have made 3 phone calls since March to the place of business and mailed a letter requesting my refund. My last phone call was placed on July 5th 2023 and I was told that I would be issued a check. Today is 8/25/2023 and I have not received my refund. I want a refund for my overpayment.
